Home Care Services

We work with service users and their representatives to ensure that they are fully briefed and aware as to when their support and care will be delivered within agreed time bands. Example of time bandings may be but not limited to the following depending on the type of agreed care package with the service-user:

  • Breakfast 06.00 to 09.30
  • Mid- Morning 09.30 to 11.30
  • Lunch 11.30 to 14.00
  • Afternoon 14.00 to 16.30
  • Tea 16.30 to 18.00
  • Early Evening 18.00 to 21.00
  • Late Evening 21.00 to 23.00
  • Waking Night 23.00 to 06.00

 

Assessment and Support Planning

Prior to the provision of a Care at Home service DCSS will ensure that it completes a full assessment of the individual Service Users’ needs.  This will be undertaken as detailed in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 (or any other successor regulations / standards).

With the appropriate consent, DCSS will ensure that  prospective and current Service Users, relatives, advocates and any relevant professional are provided with a Statement of Purpose (as detailed in the Care Quality Commission (Registration) Regulations 2009 (or any other successor  regulations / standards) and Service Users’ guide for the Provider.

A detailed risk assessment  as detailed in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 (or any other successor regulations / standards), will be completed and clearly be documented in the Service Users’ Care Plan / documentation.  Risk assessments will also be reviewed in a consistent and comprehensive basis, or should there be a change in need or change in individual circumstances.

DCSS

DCSS will ensure that there is a process for planning, implementation and evaluation of care plans which will be regularly reviewed and monitored.  The care plan will be recorded in writing and in an electronic format to show how much the service user is involved and will be reviewed as required.  Care Plans will include all details of needs of the individual service user as detailed in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 (or any other successor regulations/ standards).

DCSS will open and maintain a file in respect of each Service User; the file shall contain the following:

 

  • The Care Plan
  • Key points of Communication for the Service User
  • Key points of Communication for the Provider
  • Comprehensive Risk Assessment completed by Provider
  • Review form
  • Annual review form
  • Correspondence – records of communication relating to Service Users’
  • Communication log for all services provided by Employees at each visit that is to be read, completed and signed by the Employees attending.
  • Medical Administration Record (MAR) sheet, if required

 

DCSS will supply the Service Users with reliable  and timely information via an information pack when their Service commences and update it as required to ensure they are kept informed and involved.  The Information Pack will be user friendly, clear and understandable, and include the following:

  • Statement of Purpose
  • Contact details for the Service including out of hours and emergency contacts
  • Service provision details
  • The contingency arrangements in the event of Service interruption
  • Safeguarding information
  • How to access the Provider’s most recent CQC inspection reports
  • Complaints procedure

Live-In Home Care

We provide one to one home care and support package 24/7 that is designed to put you in total control of your personal care, housekeeping, shopping, cooking, specialist care for complex conditions, promoting and administering medication, activities of daily living and companionship. We design your plan of care in a unique way and manner to suit your needs without any timetable or routines that are rigid and delivered within the comfort of your home.

Night Care and Support

Sleep-In:

We provide well-trained care assistants that can sleep in your home by your side when they are needed by you. They have been trained not to disturb you unless you need them particularly for trips to the toilet among other necessities during the night.

Waking Night:

People with complex needs, Dementia, End of Life Care, hospital discharge after a serious accident or surgical operation may need constant monitoring of any changes in their condition during the night. Our well trained care staff are always available to monitor these changes throughout the night and make service-users comfortable during sleep.
