Furthermore, we are fully committed to person-centred values like the individuality, choice, rights, privacy, dignity, respect and partnership with service-users. Our care plan is centred on taking time to find out about an individual’s life history, preferences, wishes and needs by making sure it is written in the first person in other to own it, control it as much as possible over the choices that has been made, establish the belief that the plan is designed to make the life of the individual better and not to fit them into an existing service and to belief that an individual can plan for themselves.
We realise that the needs of our service-users can change either in the short, medium or long term and as such our care and support plans will be reviewed regularly to ensure that they are up to date and show the individual’s current needs and preferences.
